​​​​Please use the forms below to report all potential rabies exposures to humans and to domestic animals (pets) as well as submit a transfer request. The information submitted will be shared with the Fairfax County Animal Protection Police and Fairfax County Health Department Rabies Program.

Completely fill out all requested information to ensure a rapid response by public safety and public health agencies. It is required by law to report potential rabies exposures to humans and domestic animals per the Code of Virginia §3.2-6562.1.

Please call the Fairfax County Animal Protection Police at 703-691-2131 with questions regarding active events in progress, animal bites, or sick or injured wildlife. Please call the Fairfax County Health Department Rabies Program at 703-246-2433 with questions regarding the reporting system, rabies vaccinations, and active animal confinements. Rabies exposure consultation by the Health Department for physicians is available after-hours at 571-274-2296.
