Rabies Case or Confinement Transfer Request

​Submit a change in the responsible party or the confinement location for confinements that have already been put in place by an official animal control or health department agency.

By law, an amended order is required when an animal is under an order of confinement if the person or location responsible for that confinement changes.  Submitting a completed form meets that requirement.

The referenced "Confinement Case Number" and "Exposure Date" can be found on the original confinement orders provided to you by the Animal Protection Police officer.

If you are reporting a new event that involves a potential rabies exposure to a human, please complete the Human Exposure Form.  If you are reporting a new event that involves an incident between a domestic animal and wildlife, please complete the Domestic Animal Exposure Form.
